Specimen Rejection

Any specimen(s) received without proper identification (Name and Date of Birth) may be rejected. If specimen rejection would compromise patient care the following protocol will be followed: 

  • The caregiver must identify the specimen in the laboratory. The name of the caregiver will become part of the laboratory report.
  • If the caregiver can not identify the specimen in the laboratory, the ordering physician will be notified to authorize specimen processing. The name of the physician authorizing processing will become part of the laboratory report.
  • Transfusion Services (Blood Bank) will make no exceptions and will reject any specimen(s) without proper identification. A "bar-coded" Blood Bank armband number sticker must be written on the patient’s specimen label.
